Data Sharing Authorization

I authorize the City of Tucson to share my household information with SRT-authorized service partners.

FULL CONSENT — DATA SHARING AUTHORIZATION By checking this box, you authorize the City of Tucson and the SolarReady Tucson (SRT) program to collect, store, and share your household information — including your name, contact details, address, income eligibility status, housing characteristics, and program participation records — with organizations that are formally authorized by SRT to provide services to you. WHAT INFORMATION MAY BE SHARED: • Contact information (name, address, phone, email) • Household size and composition • Income eligibility documentation (type only, not amounts) • Housing characteristics (type, age, utility provider) • Program status and service history • Referral and service records WHO MAY RECEIVE THIS INFORMATION: Only organizations that have signed a Memorandum of Understanding (MOU) or equivalent agreement with the City of Tucson SRT program, and that have agreed to SRT consumer protection standards. HOW YOUR INFORMATION IS PROTECTED: • Information is shared only for the purpose of providing services to your household • Partners are prohibited from selling or using your information for marketing • You may withdraw this consent at any time by contacting SRT staff • Records are retained in accordance with City of Tucson data retention policies YOUR RIGHTS: You have the right to: request a copy of information shared about you; ask that information sharing be stopped at any time; receive services from the City regardless of whether you authorize partner data sharing (though this may limit the services available to you).
